Promotional Material/Photograph Waiver
I understand that Fon du Lac Park District. Titanium Force LLC, and Williams Tumbling LLC produce promotional materials about their programs. I understand that as a legal guardian, participant and/or spectator, I and my minor child/ward may be included in video tapes and photographs taken during the events, outings, practices and special occasions. I hereby grant Fon du Lac Park District, Titanium Force LLC, and Williams Tumbling LLC and its owners, successors, assignees, licensees and sponsors, the exclusive right to photograph or video tape myself and my minor child/ward and further utilize those materials as a part of its’ program’s, advertising and promotions without reservation or limitation. By granting this license, I understand that Fon Du Lac Park District, Titanium Force LLC and Williams Tumbling LLC and its programs are under no obligation to exercise any of its rights, licenses, and privileges herein granted by myself and my minor child/ward.
